Please complete this form if you wish to hold a one-off food stall. Do not complete this form if you are a registered mobile food business. Once you complete the form and tick the confirmations you can print off your own certificate.

Application steps:

• Read the Guidelines for Temporary Food Stalls before completing the application form
• Include as much detail as possible
• Provide an answer to every question
• You must tick all of the confirmations to allow the generation of your certificate
• Print your automatically generated certificate
• Display certificate at event.
• Contact a member of Environmental Health Team on 6238 2715 if you need assistance.
• The process should take approximately 10 minutes to complete.

Please be aware that food stalls may be inspected by an authorised officer.
If stalls are found to be non-compliant with the DHHS Guidelines for Temporary Food Stalls infringement notices may be issued.
